SP Forms Designer custom form not rendering properly.

First, let me say, "man, what a POWERFUL tool".  If I can get this to work right I will be thrilled and amazed.  Thank you to the wonderful coders who made this happen *and* are sharing it free of charge, that is KICKASS.

I have a list that I was using for Sharepoint site requests.  I've attempted to create a custom form from the SP Form Designer after successful install and activation.  However, when viewing or editing the forms they appear "truncated" and even in the source view my other fields aren't present.

I used the "Use this layout for all forms" and it shows up the same way on the display, edit, and new form radio button choices:

Then I try and create a new item using this content type and it's rendering incorrectly and is also trying to display the "Proposed Site Name" at the top which was how I had the form designed initially but have since changed it:

I tried it in Firefox thinking maybe it was an IE8 issue but the same look is present there as well.

What am I doing wrong?  Any ideas?   As I said, this is a super powerful tool and I'd love to use it if and when I can get off the ground with it!

FWIW, I was using the post at the bottom of this page as "install and use instructions" perhaps someone shoudl update it to include a note about adding the columns to your content type!

Steps to get this working:

1. install the solution
2. Activate the feature (site collection feature)
3. Edit your list settings to allow management of content types
4. Add the Modifiable Item content type
5. Change new button order and make the Modifiable Item the default content type
